The Yellow Emperor asked Shao Shi: "I have heard that people's innate constitutions vary — some are rigid and some soft, some weak and some strong, some short and some tall, some Yin and some Yang. I wish to hear the underlying principles."
Shao Shi replied: "Within Yin there is again Yin, and within Yang there is again Yang. Only when Yin and Yang are carefully examined can acupuncture have its proper standards; only when one understands where the disease originates can acupuncture have its proper order. Carefully assess the onset of illness, correspond it with the four seasons, match it internally with the five viscera and six bowels (the five viscera are Yin; the six bowels are Yang), and externally with the sinews, bones, and skin (sinews and bones are Yin; skin is Yang). Thus, there are Yin and Yang divisions within the body, and Yin and Yang divisions on the surface as well. For disease in the Yin-within-Yin, needling should target the Ying-Spring and Shu-Stream points (two categories among the Five Transport Points) of the Yin channels; for disease in the Yang-within-Yang, needling should target the He-Sea points (one of the Five Transport Points, located near the elbows and knees) of the Yang channels; for disease in the Yin-within-Yang, needling should target the Jing-River points (one of the Five Transport Points) of the Yin channels; for disease in the Yang-within-Yin, needling should target the collaterals (small superficial vessels visible on the body surface). Disease manifesting at the Yang level is termed Wind 🌬️; disease manifesting at the Yin level is termed Bi ❄️ (pain and numbness due to obstructed qi and blood); when both Yin and Yang are affected, it is called Wind-Bi. Disease with a visible prominence but no pain belongs to the Yang category; disease invisible without a prominence but painful belongs to the Yin category. When there is pain without visible prominence, the Yang aspect remains intact while the Yin aspect has been injured — one should urgently treat the Yin and not attack the Yang. When there is a visible prominence without pain, the Yin aspect remains intact while the Yang aspect has been injured — one should urgently treat the Yang and not attack the Yin. When both Yin and Yang are agitated, sometimes manifesting as prominence, sometimes hidden, accompanied by restlessness of the heart, this is called Yin prevailing over Yang — this is termed "neither exterior nor interior," and the physical body cannot endure long.
The Yellow Emperor asked Bo Gao: "I have heard that in onset, disorders of the physical form and of qi differ in sequence. How do the exterior and interior correspond to each other?"
Bo Gao replied: "Wind and cold injure the physical form; worry, fear, anger, and rage injure qi. When qi is injured, it affects the viscera, so the disease rests in the viscera; when cold injures the physical form, it manifests in the body; when wind injures the sinews and vessels, the sinews and vessels correspondingly become diseased. This is the principle of how physical form and qi correspond with exterior and interior."
The Yellow Emperor asked: "How should acupuncture be administered?"
Bo Gao replied: "For disease of nine days' duration, three needling sessions suffice; for disease of one month's duration, ten needling sessions suffice. For disease of whatever length, short or long, adjust the sessions accordingly, decreasing by this measure. For enduring Bi that does not resolve, examine the blood collaterals (small visible superficial vessels) and drain all their blood."
The Yellow Emperor asked: "For diseases of the exterior and interior, how does the difficulty or ease of treatment compare?"
Bo Gao replied: "When the physical form falls ill first and the disease has not yet entered the viscera, the number of needling sessions is halved; when the viscera fall ill first and only afterward does the physical form become affected, the number of needling sessions is doubled. This is the correspondence of difficulty and ease between exterior and interior."
The Yellow Emperor asked Bo Gao: "I have heard that the physical form can be lax or taut, qi can be exuberant or depleted, bones can be large or small, flesh can be firm or brittle, skin can be thick or thin — how does one use these to judge longevity and premature death (long life versus early demise)?"
Bo Gao replied: "When physical form and qi are proportionate, one attains longevity; when disproportionate, one suffers premature death. When skin and flesh mutually envelop each other, one attains longevity; when they fail to envelop each other, one suffers premature death. When blood and qi vessels prevail over (support) the physical form, one attains longevity; when they do not prevail over the physical form, one suffers premature death."
The Yellow Emperor asked: "What is meant by the physical form being lax or taut?"
Bo Gao replied: "When the physical form is substantial and the skin is lax and relaxed, one attains longevity; when the physical form is substantial but the skin is tight and constricted, one suffers premature death. When the physical form is substantial and the pulse is firm and large, this is favorable; when the physical form is substantial but the pulse is small and weak, this indicates qi depletion — qi depletion is dangerous. If the physical form is substantial yet the cheekbones are not prominent, the bones are small — small bones portend premature death. When the physical form is substantial and the major sinews (areas of abundant flesh) are firm with distinct striations, the flesh is firm — firm flesh portends longevity; when the physical form is substantial but the major muscles lack distinct striations and are not firm, the flesh is brittle — brittle flesh portends premature death. These are the natural laws of life, and thus one establishes the physical form and determines the qi to judge longevity and premature death. One must understand this principle thoroughly before one can face a patient and determine the outcome of life and death."
The Yellow Emperor said: "I have heard that longevity and premature death cannot be measured by any standard method."
Bo Gao replied: "When the foundation of the cheek walls (lower cheek region) is low and does not reach the level of the jaw (chin), the person will not live past thirty years; if illness is also present, they will not reach twenty years."
The Yellow Emperor asked: "How does form and qi prevailing over each other determine longevity and premature death?"
Bo Gao replied: "In ordinary people, when qi prevails over (outlasts) the physical form, one attains longevity; in illness, when the physical form shrinks and flesh is lost, if qi prevailing over the physical form brings death, and if physical form prevailing over qi brings danger."
The Yellow Emperor said: "I have heard that the needling method has three variations. What are these three variations?"
Bo Gao replied: "There is needling the Nutritive (Ying) level, needling the Defensive (Wei) level, and needling for cold Bi (numbness and pain due to cold lodged in the channels) remaining in the channels."
The Yellow Emperor asked: "What do the three variations of needling entail?"
Bo Gao replied: "Needling the Nutritive level lets out blood; needling the Defensive level emits qi; needling cold Bi aims to generate internal warmth (a sensation of therapeutic heat within the body)."
The Yellow Emperor asked: "How do diseases of the Nutritive, Defensive (Wei), and cold Bi present?"
Bo Gao replied: "In disease of the Nutritive level, there is alternating cold and heat, a lack of qi, with blood moving recklessly up and down. In disease of the Defensive level, there is qi pain that comes and goes, oppression and rumbling (with qi mechanism stagnant and rushing upward, causing abdominal sounds), with wind and cold lodged in the stomach and intestines. In cold Bi disease, the pathogen remains without leaving, causing intermittent pain and numbness of the skin without sensation."
The Yellow Emperor asked: "How does one generate internal warmth when treating cold Bi?"
Bo Gao replied: "For common people, use the fire-cautery method 🔥 (heat stimulation via fire needles or moxibustion); for distinguished persons, use the herbal fomentation method 🌿 (applying warm herbal packs)."
The Yellow Emperor asked: "How is the herbal fomentation done?"
Bo Gao replied: "Take twenty pints of unstrained wine, one pint of Sichuan pepper, one pound of dried ginger, and one pound of cinnamon bark — four ingredients in total. Bruise and grind them all, then steep them in the wine. Take one pound of silk floss and four zhang of fine white cloth, and place them together in the wine. Place the wine vessel over a slow fire of horse dung (fire maintained by smoldering dung), seal it with mud, and do not allow the vapor of the mixture to escape. After five days and five nights, remove the cloth and floss and dry them in the sun; when dry, steep them again, having them gradually absorb all the medicinal liquid — each time they must be thoroughly dried for the required number of days before being removed and dried again in the sun. After drying, take the medicinal dregs and silk floss and wrap them in double-layered cloth to make wrapped packs, six to seven chi long, making six to seven such packs. When using, heat the packs over a live mulberry charcoal fire, and apply them to the areas bearing the cold Bi that have been needled. Let the heat penetrate deep to where the disease resides; when the pack cools, re-heat it and apply it again — continue for as many as thirty applications. After sweating breaks, wipe the body with a cloth — likewise for thirty applications. Walk about indoors and be careful not to be exposed to wind. At each acupuncture session, fomentation must accompany the needling; only then will the disease resolve. This is what is called the method of generating internal heat."
