金匮要略
Someone asked: "What does it mean that a superior physician treats disease before it manifests?"
The teacher said: "What is called 'treating the disease before it manifests' means: when one sees disease of the liver, knowing that liver disease will transmit to the spleen, one should therefore first supplement and fortify the spleen. If during any of the four seasons the spleen qi is vigorous and not susceptible to pathogens, supplementing is unnecessary. An average physician, not understanding the principles of transmission among the viscera and bowels, treats only the liver when seeing liver disease, without knowing to fortify the spleen."
"If liver disease belongs to a deficiency pattern, supplement with sour flavors, assist with scorched-bitter flavors, and harmonize with sweet-flavored medicinals. Sour enters the liver, scorched-bitter enters the heart, and sweet enters the spleen. According to the five-phase generation and control cycle, spleen earth can restrain kidney water; when kidney qi is weakened, water does not move; when water does not move, heart fire qi becomes exuberant; when heart fire is exuberant, it damages lung metal; when lung metal is damaged, metal qi does not move; when metal qi does not move, liver wood qi becomes exuberant. Therefore, fortifying the spleen allows liver disease to resolve on its own. This is the essential subtlety of treating the liver by supplementing the spleen. Use this method for liver deficiency; it cannot be used for liver excess. The classics state: 'Vacuity and excess, repletion and depletion — supplement what is insufficient, reduce what is in excess,' which is precisely the meaning. Apply the same reasoning to the other viscera by analogy."
Humans receive the constant qi of the five phases and rely on the wind-qi of nature for growth and development. Wind-qi can generate all things, and it can also harm all things — just as water can carry a boat but can also capsize it. If the source-original qi of the five viscera flows freely, the person is at peace; invasion by external pathogenic wind may lead to critical illness. A thousand kinds of diseases and afflictions do not exceed three categories:
Examining these carefully, the causes of disease are all encompassed.
If one is capable of nurturing caution and not allowing pathogenic wind to invade the channels and collaterals; when it first enters the channels and collaterals but has not yet transmitted to the viscera, treat it immediately; when the four limbs first feel heavy and sluggish, use daoyin (guided exercises), tunna (breathing exercises), acupuncture, moxibustion, and ointment massage, so as not to allow the nine orifices to become blocked; moreover, do not violate the laws of the state, avoid harm from beasts and disasters, moderate sexual activity so as not to exhaust oneself excessively, regulate food and drink with respect to cold, heat, bitter, sour, acrid, and sweet, and do not allow the body to become vacuous and debilitated — then pathogenic factors will have no means to enter the couli (flesh interstices).
(Cou refers to where the triple burner meets and communicates with the source-original qi, and where blood and qi are infused; li refers to the striations of the skin, viscera, and bowels.)
Someone asked: "Patients present with facial qi-color manifestations. Please, would you explain this?"
The teacher said: "If the tip of the nose appears blue-green, there is pain in the abdomen; if the patient suffers from cold, the prognosis is poor (some say: abdominal cold with bitter pain — poor prognosis). If the tip of the nose is slightly black, there is water qi; if yellow, there is cold above the chest; if white, there is blood loss. If there is slight redness appearing at an improper season, the prognosis is poor. When the eyes are fully round (staring), this is convulsive disease (jing) and is difficult to treat."
Additionally: blue-green color indicates pain, black indicates overexertion, red indicates wind, yellow indicates difficult defecation, and a bright, gleaming complexion indicates retained fluid.
The teacher said: "If a patient is quiet and silent but suddenly cries out in alarm, the disease is in the joints and bones. If the voice is low and hoarse, unable to convey freely, the disease is in the heart and diaphragm. If the voice is thin and drawn-out, the disease is in the head."
When respiration raises the shoulders, there is hardness and fullness in the center (heart region). When respiration draws upon the chest, there is qi rushing upward with coughing, mouth open with short breathing — this is lung wilting (fei wei) with spittle and foam.
When inhalation is slightly rapid, the disease is in the middle burner and belongs to an excess pattern — purgation will cure it; cases of deficiency are difficult to treat. In the upper burner, inhalation is short and urgent; in the lower burner, inhalation is deep and prolonged — both are difficult to treat. When the body shakes and trembles with respiration, this is difficult to treat.
The teacher said: "The pulse at the cunkou (inch-mouth) moves in accordance with the prevailing season of dominance. For example, when the liver is dominant, the complexion should be blue-green, and in each of the four seasons the complexion should match its corresponding color. If a liver-dominant person has a green complexion but instead appears white, then off-season colors and pulses both indicate disease."
Someone asked: "What is meant by: 'Arriving before it should, arriving but not arriving, arriving and not departing, and arriving in excess'?"
The teacher said: "After the winter solstice, at midnight of the jiazi day, the shaoyang qi begins to rise. At that time, yang qi is newly born, and the weather should turn mild and warm. If the weather is already warm before the jiazi day has come, this is 'arriving before it should.' If at the arrival of the jiazi day the weather is not yet warm, this is 'arriving but not arriving.' If at the jiazi day the great cold remains unresolved, this is 'arriving and not departing.' If at the jiazi day the weather is warm like the fifth or sixth month of summer, this is 'arriving in excess.'"
The teacher said: "When a patient's pulse is floating at the cubit (chi) position, the disease is external (in the surface); when the pulse is floating at the foot (chi) position, the disease is internal. This may be seen in lumbar pain, back stiffness, inability to walk, with inevitable shortness of breath and extreme fatigue."
Someone asked: "The classics state: 'Jueyang moves alone.' What does this mean?"
The teacher said: "This describes the presence of yang without yin; hence it is called jueyang (reversed yang)."
The teacher said: "When the cun pulse is deep, large, and slippery: depth indicates repletion, slippery indicates qi; repletion and qi act together, and if blood and qi enter the viscera, the prognosis is poor; if they enter the bowels, there will be recovery. This is called sudden reversal (cu jue). What does this mean? If the lips and mouth turn blue and the body becomes cold, this is entering the viscera — poor prognosis; if the body is warm and sweating spontaneously, this is entering the bowels — good prognosis."
Someone asked: "When the pulse disappears ('pulse collapse'), entering the viscera means death, and entering the bowels means recovery. What does this mean?"
The teacher said: "This does not apply to only one disease — to all hundred diseases it is the same. For example, with **spreading sores (jin yin chuang)*: if they spread from the mouth toward the four limbs, they are treatable; if they spread from the four limbs toward the mouth, they are difficult to treat. Disease external is treatable; disease entering the interior carries a poor prognosis."
Someone asked: "What are the eighteen yang diseases?"
The teacher said: "Headache, stiff neck, lumbar pain, back pain, arm pain, and foot pain with pulling spasms."
"What are the eighteen yin diseases?" The teacher said: "Cough, counterflow qi ascent, wheezing, hiccup, throat problems, intestinal rumbling, abdominal distension, heart pain, and spasmodic tightness."
The five viscera diseases each have eighteen, combined making ninety diseases. Humans also have six subtle conditions, with each subtlety having eighteen diseases, making one hundred eight diseases in total. The five lasses, seven damages, six extremes, and the thirty-six diseases of women are not included within this scheme.
Clear (light) pathogens mostly attack the upper body; turbid (heavy) pathogens mostly attack the lower body. Greater major pathogens mostly strike the exterior; lesser minor pathogens mostly strike the interior. Pathogens from food and drink that enter through the mouth cause retained food. The five pathogens each strike a person following their own patterns: wind pathogen strikes in the morning (beginning), cold pathogen strikes at dusk, dampness injures the lower body, fog/mist damages the upper body. Wind makes the pulse floating, cold makes the pulse tight, fog harms the skin, dampness flows to the joints, food harms the spleen and stomach, extreme cold injures channels, and extreme heat injures the collaterals.
Someone asked: "There are situations where medicine says to rescue the interior urgently, or to rescue the exterior. What does this mean?"
The teacher said: "When a patient has received erroneous purging by a physician and subsequently develops unremitting diarrhea with undigested food, combined with body pain — rescue the interior urgently. When the body pain persists but bowel movements have returned to normal — rescue the exterior urgently. When there is a chronic disease aggravating an acute condition, first treat the acute disease, then attend to the chronic condition."
The teacher said: "Disease of the five viscera resolves when each viscus obtains what it has an affinity for; each viscus also has what it disdains; each viscus creates disease according to what it does not find agreeable. If a patient who normally does not care for a certain food suddenly craves it intensely, fever will surely develop. In all disease located within the viscera, if one needs to attack the pathogen, one should attack according to where the pathogen has settled ('what has been gained'). For example, with thirst accompanied by a water-heat binding pattern, one uses Poria Five Powder (Zhuling Tang) in the classical approach — this ancient formula promotes urination and clears heat, exemplifying the therapeutic principle. All other patterns follow this same reasoning."
