If malaria attacks on the first day of each lunar month, the ancients believed it should generally resolve by the fifteenth day; if it does not improve, it should ease by the end of the month. If it still does not resolve, the teacher said: this indicates that the pathogen has lingered long and become congealed into a mass (fixed abdominal masses are called "zheng"; mobile ones are called "jia"), known as the malarial mother. It should be treated promptly, and Biejia Jian Wan (Turtle Shell Decocted Pill) is appropriate.
The classical formula for Biejia Jian Wan consists of twenty-three ingredients: turtle shell, blackberry lily rhizome, skullcap root, bupleurum root, pillbug, dried ginger, rhubarb, peony root, cinnamon twig, tingli seed, pyrrosia leaf, magnolia bark, tree peony root bark, broken rice flower, aster flower, pinellia tuber, ginseng, ground beetle, donkey-hide gelatin, wasp nest, mirabilite, dung beetle, and peach kernel. The classical method involves soaking lime from a forge hearth in clear wine, then placing the turtle shell in this mixture and cooking until it becomes like glue or lacquer. The juice is squeezed out, then added to the other herbs and decocted into pills the size of wutong tree seeds. Seven pills are taken on an empty stomach in the morning, three times daily. The formula recorded in later editions of the Qianjin Fang differs slightly, such as adding seaweed and euphorbia, and omitting pillbug and mirabilite. The composition and usage of this formula are solely records from ancient literature, for study purposes only, not as medication guidance.
The teacher said: When yin qi is isolated and exhausted, and yang qi alone flares, there will be fever, shortness of breath, vexation in the chest and heart, heat in the hands and feet, and frequent nausea toward vomiting. Warm malaria presents with a pulse that appears close to normal, the body has no chills but only fever, with painful and aching joints and frequent vomiting. The classical formula used is Baihu plus Guizhi Decoction (White Tiger plus Cinnamon Twig Decoction) . This formula consists of anemarrhena rhizome, licorice root, gypsum, rice, and cinnamon twig. It was decocted and administered according to classical methods, and sweating indicates recovery.
Malaria with predominantly cold symptoms is called mu malaria (female-type malaria) , and the classical formula used is Shuqi Powder. Shuqi Powder consists of three equal parts of dichroa root (shuqi), mica, and dragon bone.
