Chapter 21 of 78
黄帝内经·素问
Holistic Interpretation
The Yellow Emperor asked: Do people's living environments, activity states, and courage or timidity cause changes in the pulse manifestations?
Qi Bo replied: Generally speaking, fright, anger, fatigue, activity, or stillness can all cause changes in the pulse. For example:
Therefore, it is said: The essential principle of diagnosis lies in observing a person's courage or timidity, and the firmness or softness of their bones, flesh, and skin. In this way, one can understand the condition of the illness—this is the fundamental rule of diagnosis.
Observe further the locations and causes of sweating:
Thus, amidst the yin-yang changes of the four seasons—spring, summer, autumn, and winter—illness often arises from overexertion (exceeding the body's tolerance limit). This is a universal principle.
Distribution of Essence from Water and Grain:
🍚 Food enters the Stomach and, after digestion, the essential substances (nutritive components) are dispersed to the Liver, which then nourishes the sinews throughout the body.
After food enters the Stomach, the heavier portion (turbid qi) is attributed to the Heart which then infuses its essence into the blood vessels. The qi of the vessels flows through the meridians, and the qi of all meridians converges in the Lung—the Lung faces and governs all vessels (it gathers the qi of all meridians of the body)—and transmits the essence to the skin and body hair. The essence qi of the skin and hair merges with that of the meridians and flows to the fu (points where meridians converge). When the fu has abundant essence qi, the spirit is clear and bright, retained in the four viscera (the Heart, Liver, Spleen, and Kidney), and the body's qi attains balance and harmony (balance/equilibrium). Once qi is balanced, it is reflected at the qi opening (the wrist, the inch pulse), forming the pulse manifestation by which life and death can be determined.
💧 Water enters the Stomach; its essence fills up and is transmitted upward to the Spleen. The Spleen disperses this essence, returning it upward to the Lung. The Lung then regulates the water passages (dredges and regulates the pathways through which water flows) and sends it downward to the Bladder. The essential of water spreads throughout the entire body, flowing in parallel with the wiris ofthe five viscera, acting in accordance with the patterns of the four seasons, the five viscera, and yin and yang. This is the normal standard of water metabolism.
Manifestations of Excess in Particular Meridians and Their Regulation:
☀️ Excess of the Greater Yang meridian (Taiyang): Manifested as qi reversal with panting and deficiency qi rising upward. This represents insufficiency of yin with excess of yang. Both the exterior and interior channels (Greater Yang and Lesser Yin) should be treated with reducing methods, needling the lower transport points (the shu points on the foot).
🌕 Excess of the Sunlight Yang meridian (Yangming): This is double yang (yang qi excessively concentrated in Yangming). Treatment should tonify yin and reduce yang, selecting the lower transport points.
🌓 Excess of the Lesser Yang meridian (Shaoyang): This is due to rebellious qi. Sudden swelling appears in front of the ankle (above the qiao). Treatment uses the lower transport points. Excessive Shaoyang indicates an excess of one yang qi.
🌑 Abnormal beating of the Greater Yin meridian (Taiyin): The physician must carefully examine the true visceral pulse (differentiate the true manifestation of the viscera). If the qi of the five vessels is diminished and the stomach qi is not harmonious, the cause lies with the three yin (Greater Yin). Treatment should focus on its lower transport points, tonifying yang and reducing yin.
🌗 Unusual surging single vibration of the Lesser Yang (Shaoyang): This is rebellious qi of Lesser Yang. Yang qi ascends in conjunction. The four arteries (the remaining four vessel systems) compete to expand, and the qi returns to the Kidney. Treatment should address its meridians and collaterals, reducing yang and tonifying yin.
🌘 The Sole Yin (Jueyin) arrives just alone: This is governed by the Sole Yin meridian. True yin stirs at the level of the Heart. Rebellious qi lodges itself and clumps, manifesting in white sweat (cold sweat). One should adjust diet and combine it with medication, with treatment at the lower transport points.
The Yellow Emperor asked: What is the pulse manifestation of the Greater Yang meridian?
Qi Bo replied: It manifests as the aspect of three yang (the most exuberant yang), presenting a floating pulse.
The Yellow Emperor asked: What is the pulse manifestation of the Lesser Yang meridian?
Qi Bo replied: It manifests as the aspect of one yang (yang at its initial rise), being slippery but not substantial (the pulse arrives slick and slippery but lacks underneath fullness).
The Yellow Emperor asked: What is the pulse manifestation of the Sunlight Yang meridian?
Qi Bo replied: The pulse appears large and floating. When the Greater Yin meridian pulsates, the pulse appears hidden yet drum-like (deep-lying but thrusting). When the two yin (Lesser Yin) arrives with a strong beating, then the Kidney pulse is deep and not floating.
The central scholarly concept of this treatise lies in these five words: "Illness arises from overexertion." Threaded through the discussion of emotion, rest and fatigue, diet, water metabolism and meridian imbalances is this single thread: the human body is a dynamic equilibrium system, and any strain when functions are in any way overloaded or overstimulated — will shatter the balance and induce disease pathology.
In concrete terms, the text presents three principal ideas:
First, "observe courage/determination in diagnosis" —Individual Differences Determine Whether Illness Occurs. When the human brain faces the same combination of trauma, fear, and toil, "the vigorous dispel through their qi; the passive harbor restlessness and sicken." Qi Bo posited that factors arguably independent of functional mental context: each individual's inborn physique or tendency toward courage or resignation — set immovable milestones The outcome the factors do or do not take root in When our perceptions identify, strong endogenous strength is not a cliché.
Second, "Distinguishing Sweat Origin to Reveal Organ Dysfunction" — Reverse Differential mapping Anatomical vs. physiological from empirically assessed profiles. Who are Actually of sweat between anatomical(those working belly = Greater); based The Different Causation which within etc., due possibly based the origin A dry day? This thereby showed patho-clusters Diagnosis body-area types due Different syndromes view of correlativity rather on factors baseline(health not) such an mapping reflects sinological total unity architecture "body surface-correspondences-emotion-activity".
Third, "All Meridians Converge in the Lung–Thus Inch-Level Pulse Quantifies Visceral Vitality": The Root Theorem enabling Pulses Diagnoses. Once ingested food essence and juice of transformed digestia after "Greater Gastric / Energy … passages., centrifugal staging – reach Axilla overthrough to make P∞ at Qt–Which via a concentration: This is lung gauge point flows all — this why much later" merely feeling radial, today three fingers measured at one pulse site diagnose almost every course or system ofall function-bearing networks, yet for the ancestor paradigm for &"Single-location includesReadWhere single Wrist Sample Infers." whole—offspring.
Fourth, Core Regulating Principle of "R:Subdual=F full→Evacuation&–Recessité apply Supplement-Reś." If set Extremes or hyper via divergence specific region,(four patterns) method correct actions should分 balancing relative YIN&ō strengthen sequence possible. State law "sympt.R".
