The Yellow Emperor asked: The occurrence of various diseases is certainly caused by external factors such as dryness, dampness, cold, summer heat, wind, and rain; and internal factors such as yin-yang disharmony, emotional disturbances of joy and anger, irregular diet, and improper living conditions. When pathogenic qi combines with healthy qi, a diseased condition forms. When pathogenic qi invades a specific viscus, the disease obtains a name. I already understand this. Yet in most diseases, the patient feels refreshed in the morning, stable during the day, aggravated in the evening, and most severe at night. Why is that?
Qi Bo replied: This is the influence of the qi of the four seasons.
The Yellow Emperor said: I would like to hear you explain the principle of the qi of the four seasons.
Qi Bo said: Spring engenders, summer grows, autumn harvests, winter stores—these are the normal movements of the qi of the four seasons, and the human body corresponds to them. If one divides a day into four seasons: morning corresponds to spring, midday to summer, sunset to autumn, and midnight to winter. In the morning, the yang qi of the human body begins to rise, and pathogenic qi declines, so the patient feels refreshed in the morning (dan hui). At midday, the yang qi of the body is exuberant, and flourishing yang qi can overcome pathogenic qi, so the patient is stable (zhou an). In the evening, yang qi begins to constrict and decline, and pathogenic qi begins to grow, so the condition worsens in the evening (xi jia). At midnight, yang qi is hidden and stored within, and pathogenic qi alone permeates the body, so the patient's condition is the most severe (ye shen) at night.
The Yellow Emperor asked: Yet in some cases, the condition does not follow this pattern. Why?
Qi Bo replied: This occurs when the disease does not respond to the qi of the four seasons but is dominated by a single viscus. If the disease is not influenced by the four seasons but is solely dominated by visceral qi, it will worsen at the time when the viscus is overcome (the time of its counter-restraining agent), and it will subside at the time when the viscus overcomes another (the time of its restraining agent).
The Yellow Emperor asked: How should this be treated?
Qi Bo replied: Follow and conform to the patterns of Heaven with appropriate measures of regulation and treatment, and recovery can be anticipated. A practitioner who can conform to Heaven is an excellent practitioner (gong); a practitioner who defies Heaven is a coarse practitioner (cu).
The Yellow Emperor said: Excellent. I have heard that the acupuncture method has the “Five Transformations” (the five corresponding variations of the five zang viscera), used to correspond to the Five Shu Points (the five categories of well, spring, stream, river, and sea). I would like to hear you explain the underlying principle.
Qi Bo replied: Human beings have five zang viscera; each of the five viscera has five transformations; and each of the five transformations has its corresponding point among the five Shu. Thus, five times five gives twenty-five points, corresponding to the five tastes.
The Yellow Emperor said: I would like to hear you explain the Five Transformations.
Qi Bo said: The liver is a “Mu (male/yang) viscus”; its color is green, its season is spring, its days are jia-yi, its sound is jue, its taste is sour. The heart is a “Mu (male/yang) viscus”; its color is red, its season is summer, its days are bing-ding, its sound is zhi, its taste is bitter. The spleen is a “Pin (female/yin) viscus”; its color is yellow, its season is long summer, its days are wu-ji, its sound is gong, its taste is sweet. The lung is a “Pin (female/yin) viscus”; its color is white, its season is autumn, its days are geng-xin, its sound is shang, its taste is acrid. The kidney is a “Pin (female/yin) viscus”; its color is black, its season is winter, its days are ren-gui, its sound is yu, its taste is salty. These are the “Five Transformations”.
The Yellow Emperor asked: How do the Five Transformations correspond to the Five Shu points?
Qi Bo replied: The five viscera govern winter; when disease lies in a viscus, take the “Well” (Jing-well) point. The five colors govern spring; when disease manifests in staining, take the “Spring” (Ying-spring) point. The five seasons govern summer, so when the condition is sometimes heavy and sometimes light, take the “Stream” (Shu-stream) point. The five sounds govern long summer, so when the disease has changed in sound, take the “River” (Jing-river) point. The five tastes govern autumn; when the disease is in the stomach, governed by the stomach due to improper diet, take the “Sea” (He-sea) point. This is the law of the five transformations corresponding to the five Shu points.
The Yellow Emperor asked: How do the “Source” (Yuan-source) points combine, so that together they form six Shu points?
Qi Bo replied: The Yuan-source points do not uniquely correspond to the five seasons but are integrated with the “River” points to match the principle of five, so there are six times six, thirty-six Shu.
The Yellow Emperor asked: Why is it said that the viscera govern winter, the seasons govern summer, the sounds govern long summer, the tastes dominate autumn, and the colors govern spring? I would like to hear the reasoning.
Qi Bo replied: In diseases lying in the five viscera, one need take the “Well” points. For pathological transformations appearing in the facial colors, take the “Spring” points. For conditions that are alternatively heavy and light, take the “Stream” points.
