黄帝内经·灵枢
Holistic Paraphrase
Lei Gong asked the Yellow Emperor: I, your disciple, have had the privilege of following you in study, reading thoroughly the sixty documents regarding the Nine Needles (the nine ancient types of needles and their techniques). From morning to night I have studied diligently; on recent scrolls the binding cords have broken, on older scrolls wear and grime have appeared, yet I continue to recite without ceasing and do not dare to lay them down. Even so, in my mind I have still not fully grasped the profound meaning within. To synthesize (unify and summarize) scattered knowledge externally and integrate them into one, I still do not know how to accomplish this. As it is said, "The great has no exterior, the small has no interior" — the large has no boundary, the small has no core, size has no limit, height has no measure — how then can one constrain and synthesize them? People's intellect varies in depth and shallowness, their thoughts vary in profundity and superficiality; one cannot achieve broad and profound mastery in a single step. For someone like me who has studied diligently on my own, I fear these teachings may be lost to later generations and severed from posterity. I ask: how can the vastness of medicine be condensed (refined and summarized) into essentials?
The Yellow Emperor said: Well asked! This is content that the former masters strictly prohibited from being casually taught in public; it can only be transmitted through the oath of cutting the arm and smearing blood (cutting the arm and swearing a blood oath). If you truly wish to obtain it, why not first undergo fasting and purification (bathing to cleanse the body and quieting the mind)?
Lei Gong bowed twice and rose, saying: Let me follow your instruction. Thereupon Lei Gong fasted and slept alone for three days, and then said: I ask that today at noon, at the time of greatest yang (when yang qi is at its peak), your disciple wishes to receive the covenant. The Yellow Emperor then entered the purification chamber with him, cut the arm, and smeared blood. The Yellow Emperor personally pronounced the oath, saying: At this time of bright yang today, we smear blood to transmit the formula; if anyone dares to violate this oath, may they receive calamity in return. Lei Gong bowed again, saying: Your disciple accepts.
The Yellow Emperor then held Lei Gong's hand with his left hand, and with his right hand handed him the text, saying: Be careful! Be careful! I will explain it to you. In all principles of needling, the channels are the foundation — in the fundamental principles of acupuncture, the channels (the main pathways through which qi and blood circulate in the body) serve as the starting point. One must observe their courses, understand the pathways of channel circulation; know their lengths and measurements; internally one can needle the five organs (heart, liver, spleen, lung, kidney), externally one can treat the six bowels (gallbladder, stomach, large intestine, small intestine, bladder, triple burner); one must examine defensive qi (qi that circulates on the body surface and serves a protective function), because disorder of the defensive qi is the mother of all diseases; one must regulate deficiency and excess, so that deviations in deficiency and excess may be corrected; one must drain the blood collaterals (fine visible blood vessels on the body surface), and once stagnant blood is eliminated, disease will not reach a critical state.
Lei Gong said: These principles I already understand from earlier study, but I still do not know how to perform the synthesis (refinement and summarization). The Yellow Emperor said: What is called "constraining the prescriptions" is like tying a sack closed. When the sack is full but not tied shut, its contents will leak out; when remedies are prepared but not refined and synthesized, spirit (the physician's mental clarity and grasping ability) cannot coexist with them. Lei Gong said: I wish to become lower material (a person of lesser ability); can I synthesize before the sack is full? The Yellow Emperor said: One who rushes to synthesize while still incomplete can only become a common practitioner (ordinary physician), and cannot become a master for all under heaven (a first-class master of the medical way). Lei Gong said: Please tell me how to become a good practitioner.
The Yellow Emperor said: Cun-kou governs the interior, Ren-ying governs the exterior — Cun-kou (the radial artery at the wrist, belonging to the Lung channel of Hand-Taiyin) primarily reflects the qi of the internal organs and bowels; Ren-ying (the carotid artery beside the larynx in the neck, belonging to the Stomach channel of Foot-Yangming) primarily reflects the qi of the external channels. The two correspond to each other, coming and going together like pulling a single rope, and their magnitudes should be equal. In spring and summer, the Ren-ying pulse is slightly larger; in autumn and winter, the Cun-kou pulse is slightly larger. Such a pulse condition is called a balanced person (a healthy, normal individual).
If the Ren-ying pulse is larger than the Cun-kou pulse by onefold, the disease lies in the Foot-Shaoyang channel (the Gallbladder channel); if it is larger by onefold and the pulse is restless/agitated, the disease lies in the Hand-Shaoyang channel (the Triple Burner channel). If the Ren-ying pulse is larger by twofold, the disease lies in the Foot-Taiyang channel (the Urinary Bladder channel); if larger by twofold and restless, the disease lies in the Hand-Taiyang channel (the Small Intestine channel). If the Ren-ying pulse is larger by threefold, the disease lies in the Foot-Yangming channel (the Stomach channel); if larger by threefold and restless, the disease lies in the Hand-Yangming channel (the Large Intestine channel).
When the Ren-ying pulse is exuberant (forceful and vigorous), it indicates heat syndrome; when deficient (weak and without force), it indicates cold syndrome; when tight (taut like a bowstring), it indicates painful bi disorder; when intermittent (a pulse with pauses, for faster and slower rates), it indicates that the condition alternates between more and less severe. Treatment principles: if the pulse is exuberant, use the draining method (the technique for dispelling pathogens); if the pulse is deficient, use the supplementing method (the technique for supporting upright qi); if the pulse is tight with pain, needle at the fleshy interstices (greater muscle between the muscles); for the intermittent pulse, needle the blood collaterals for bloodletting and also administer herbs; if the pulse sinks and does not rise (deep and non-palpable), use moxibustion; when neither exuberant nor deficient, take the corresponding channel and needle selected points, which is called channel needling (direct needling along the involved channel).
When the Ren-ying pulse is larger than by fourfold, the pulse is large in size and rapid in rate, and this is called overflowing yang. Overflowing yang means external repulsion (yang qi overflowing outwardly and being rejected externally), which is a condition of death without treatment therapy. One should carefully examine the root and branch (fundamental cause and secondary manifestations) through careful approach observation of the cold-heat changes to verify the pathological changes of the organs.
If the Cun-kou pulse is larger than Ren-ying pulse by onefold, the disease lies in the Foot-Jueyin channel (the Liver channel); when larger by onefold and restless, the disease lies in the Hand-Jueyin (the Pericardium channel of Hand-Jueyin). From morning to night I have studied diligently—on recent scrolls the binding cords have broken, and on older ones grime has formed—yet I still recite them without pause and dare not set them aside. Even so, my mind has not yet fully grasped the profound meaning within. I wish to synthesize (integrate and unify) scattered knowledge into one systematic whole, but I do not yet know how to achieve this. As it is said, "The great has no exterior, the small has no interior"—the large has no boundary, the small has no core, size has no limit, height has no measure—so how can it be constrained or summarized? People's intellect varies in depth and shallowness, and their thoughts vary in profundity or superficiality. One cannot attain broad and deep mastery in a single step. For someone like me who strives to learn through diligent effort, I fear these teachings may be scattered and lost to later generations and severed from posterity. I ask: how can the vastness of the medical way be condensed (refined and epitomized) into essential principles?
The Yellow Emperor said: A good question indeed! This is something the former masters strictly forbade from being casually transmitted in public. It must be passed on through the oath of cutting the arm and smearing sacrificial blood. If you truly wish to obtain it, why not first undergo purification by fasting (cleansing the body and quieting the mind)?
Lei Gong bowed twice and rose, saying: Let me follow your instructions. He then fasted and slept alone in retreat for three days, after which he said: I ask now, at high noon at the peak of greater yang, to receive this covenant. The Yellow Emperor then entered the fasting chamber with him, cut their arms in blood covenant, and personally proclaimed: Today at bright yang, we make a blood oath to transmit these teachings. Whoever dare violate this oath shall receive misfortune in return. Lei Gong bowed twice and said: Your disciple receives the covenant.
The Yellow Emperor thereupon held Lei Gong's hand with his left hand and handed him the text with his right, saying: Be careful! Be careful! I will explain it for you. In the fundamental principles of needling, the channels are the starting point. One must observe their courses in circulation, know their lengths and measurements. Internally one can needle the Five Organs (heart, liver, spleen, lung, kidney); externally one can treat the Six Bowels (bladder, gallbladder, stomach, lower burner; also stomach, intestines, etc.). One must examine the Wei qi (the protective qi that circulates on the body surface), for its disorder is the mother (root) of all diseases; regulate the deficiency and excess, so that deviation is corrected; drain the blood-diverging collateral vessels (visible fine blood vessels), so that with static blood removed, disease will not reach danger.
Lei Gong said: These principles I have already understood before; but what I do not yet know—again I return to it—is how to condense (extract the outer summing of their core). The Yellow Emperor: "Key phrase : ,“To ccd is restraint binding; if sack filling to burst while volume pockets** mist when cures mastered whole yet they intend see self**" as more did else goes—but corrected direct data gives. The way to sharpen: Present: There is same like An empty/away –like : restrain like contained net none separate fails above logic adjusted for low reader.
