Overall Interpretation
The Yellow Emperor asked: When doctors treat patients, for the same illness, different doctors adopt different treatments, yet all can cure. Why is this so?
Qi Bo replied: This is due to differences in the geographical environment. 🌍
The eastern region is where the qi of Heaven and Earth begins to arise. It borders the sea and abounds in fish and salt. The local people there subsist mainly on fish and favor a salty taste. They are content with their dwellings and satisfied with their food. Eating too much fish causes accumulated heat 🔥 to arise within the body, and excessive salt consumes and damages the blood 🩸. Thus the local people mostly have dark skin and loose pores, and they are prone to carbuncles and ulcers (purulent infections of the skin and subcutaneous tissues). For treating such illnesses, bian stone (an ancient sharp stone medical tool used for lancing pus or bloodletting) is most suitable. Therefore, the therapy of bian stone originated from the East.
The western region abounds in gold and jade, with sand and stone everywhere. It is the place where the qi of Heaven and Earth converges and descends. The local people dwell in hilly terrain where winds are frequent and strong, and the soil and water are hard and tough in nature. They are not particular about fine clothing, wearing coarse wool and spreading straw mats. Their food is rich and greasy, and their bodies are fat and stout. For this reason, external pathogens find it difficult to attack their physical forms, and their ailments arise mostly from within (such as food stagnation and visceral dysfunction). For treating these ailments, medicinal herbs (du yao) (the ancient general term for potent and strongly biased drugs) are most suitable. Therefore, herbal therapy originated from the West.
The northern region is a place of closure and storage, where the qi of Heaven and Earth seals and hides. The terrain is high and steep, wind is cold and piercing, and ice and frost prevail. The local people delight in outdoor activities in the fields and take dairy products as their main food. Exposure to prolonged cold makes the viscera prone to cold, producing conditions such as abdominal distension. For treating these ailments, moxibustion (jiu ruo) (use of burning mugwort to apply heat at acupoints or affected areas) is most suitable. Therefore, moxibustion therapy originated from the North.
The southern region is where the qi of Heaven and Earth nourishes all things and yang qi is at its peak. The terrain is low-lying and damp, the water and soil are soft and weak, and fog and dew are heavy. The local people favor sour foods and fermented foods. As a result, their pores are fine and dense, and their complexion tends to be reddish. They are prone to contraction and atrophy (luan bi) (a category of diseases involving tendon spasms, numbness and pain in the limbs). For treating these ailments, fine needles (wei zhen) (tiny needle tools, which later developed into the Nine Needles system) are most suitable. Therefore, acupuncture therapy originated from the South.
The central region is flat, damp, and abundant, the place where the qi of Heaven and Earth transforms and generates the most things. The local people have a wide variety of foodstuffs, but their physical activity is limited. Hence their ailments are predominantly wilting and retrograde (wei jue) (weakness and atrophy of the limbs, reversal and chaos of qi and blood) and cold and heat (conditions characterized by alternating fever and chills). For treating these ailments, daoyin introspective rubbing (daoyin anqiao) (methods of moving the body, regulating breathing, and self-massage to unblock qi and blood) is the most suitable. Therefore, the therapy of daoyin and anqiao originated from the Central Region.
Thus, sage and enlightened persons are able to integrate all kinds of therapies, selecting the most appropriate method according to each patient's specific circumstances. Though the treatments may differ, the ailments are all cured, because the physician understands the essence of the illness and has mastered the great principle of treatment. 🌿
