The Yellow Emperor asked: I have heard that the human body has three hundred and sixty-five qi acupoints (surface apertures where qi and blood enter, exit, and converge), corresponding to the three hundred and sixty-five days of the year, but I do not know their specific locations and wish for your detailed explanation. Qi Bo bowed and replied: This question is indeed profound! Were it not for a sage ruler, who could fathom its principles? Please allow me to explain these acupoint locations as thoroughly as possible. The Yellow Emperor cupped his hands, stepped back, and said humbly: Master, you have opened up my thinking; though I have not yet seen the acupoint locations with my own eyes nor heard their numbers with my own ears, I already feel my eyes brightened and my ears sharpened. Qi Bo said: This is what is meant by "the sage is easy to enlighten, the fine horse is easy to guide." The Yellow Emperor said: I am not an easy sage to enlighten; people say that the "true numbers" (genuine and crucial acupoint patterns) can open one's mind, and what I am asking about now are precisely the "true numbers" — this is still only initial enlightenment and dispelling of confusion, not yet in-depth exploration. Nevertheless, I still hope you will fully explain these locations, allowing me to grasp their meaning. I shall treasure them in a golden cabinet and not lightly divulge them.
Qi Bo bowed again and said: Allow me to explain. When the back and the chest/heart mutually pull and cause pain, the acupoints to be taken are Tiantu, the tenth vertebra, as well as Shangji and Xiaji. Shangji is the stomach duct area (Zhongwan), Xiaji is Guanyuan. The back and chest channels connect obliquely with yin and yang on both sides, so this disorder causes front-and-back pain, astringency and blockage, chest and rib pain with labored breathing, inability to lie flat, qi rushing upward, shortness of breath, one-sided pain, tense and protruding pulse, with the channel running obliquely to the coccyx, connecting with the chest and ribs, supporting the chest and heart, piercing the diaphragm, ascending to the shoulder to connect with Tiantu, and obliquely descending from the shoulder to converge below the tenth vertebra.
The five viscera have fifty efferent acupoints total, the six bowels have seventy-two. The acupoints for heat disorders number fifty-nine, those for water disorders number fifty-seven. Water acupoints are distributed among the various intermuscular spaces; heat acupoints lie at the sites of qi cavities; cold-heat acupoints are the two in the two hip shafts; the great prohibition consists of twenty-five acupoints located five cun below Tianfu. Together these amount to three hundred and sixty-five acupoints, which are the sites where needling is carried out.
The Yellow Emperor said: I now know the locations of the qi acupoints and also understand the sites where needling travels and moves in and out; I would also like to hear whether the collaterals (minute superficial vessels) and the streams and valleys (intermuscular gaps) have corresponding features? Qi Bo said: The minute collaterals possess three hundred and sixty-five acupoint meetings, also corresponding to the year, used for discharging abnormal pathogenic qi and ensuring the circulation of nutritive and defensive qi (nutritive qi moving within the vessels, defensive qi outside). If nutritive and defensive qi stagnates, defensive qi dissipates and nutritive blood spills outward, or qi becomes exhausted and blood stalls, the exterior then manifests fever while the interior manifests shortness of breath. One should rapidly apply the draining method, without idleness, in order to unblock the nutritive and defensive systems. Whenever stagnation is seen, drain it, without insisting that it be exactly upon a designated acupoint.
The Yellow Emperor said: Good. I would also like to hear about the convergence of the streams and valleys. Qi Bo said: The great meeting points among the muscles are called "valleys," and the smaller meeting points are called "streams." Between the interstices of the muscles and sinews lie the meeting points of streams and valleys, which serve to circulate nutritive and defensive qi and to converge the great qi. If pathogenic qi overflows and qi becomes blocked and congested, the vessels become hot and the flesh putrefies; nutritive and defensive qi failing to move will transform into pus, internally eroding marrow and bone and externally destroying large masses of muscle; if it lingers at the joint spaces and skin interstices, it will bring about deterioration and decay. If cold pathogenic factors gather and stay, nutritive and defensive qi cannot abide peacefully, the muscles contract, sinews constrict, and the arms and elbows cannot extend; internally one may develop bone bi (pain and dysfunction of bones and joints), externally one may develop numbness and insensitivity of the skin and flesh, together referred to as deficiency, caused by great cold lodging in the streams and valleys. The streams and valleys possess three hundred and sixty-five acupoint meetings, likewise corresponding to the year. If small bi-pathogens spread and migrate, moving back and forth along the vessels, one can treat them with fine needling, following thereby just as in the ordinary principles of needling.
The Yellow Emperor, having then dismissed his attendants, rose and bowed twice, saying: Today I have received enlightenment and dispelling of confusion; I shall treasure this in the golden cabinet and not lightly transmit it, and further place it in the Golden Orchid Chamber, giving it the title "Location of Qi Acupoints." Qi Bo said: In the small collateral vessels with separate outward tracks from the main channels, if the blood is superabundant and draining is appropriate, there likewise are three hundred and sixty-five pathways, all of which inject into the major collaterals and through them transmit into the twelve main collateral systems — not only through the fourteen main collaterals; those that internally disperse and drain into the middle burner consist of ten vessels.
